Full-Service Vision Plan
Comprehensive benefits at an affordable cost are what you will find in our Full-Service Vision Plans. These plans provide routine examinations and eyewear, with a choice of deductibles and frequency of services, and can be written in conjunction with a health coverage or on a stand-alone basis.
Eyewear-Only Plan
Because some health maintenance organizations and preferred provider organizations already provide coverage for routine eye examinations, we offer an Eyewear-Only Plan for groups with fifty (50) or more employees. Savings are realized by eliminating duplicate examination coverage.
Administrative Services
We provide professional management for employers who elect to self-insure a Full-Service or Eyewear-Only Vision Plan. Included in our very competitive fee structure are the most comprehensive management information reports in the vision care industry.
Voluntary Vision Plan
Groups with fifty (50) or more employees may offer a voluntary or “cafeteria” plan. A minimum of twenty (20%) percent of eligible employees are required to participate.
Discount Vision Program
Our Discount Vision Program offers a twenty (20%) percent discount off retail charges for routine eye examinations, lenses, frames, and optical accessories. It is available on an individual basis, or can by sponsored by an Association, Trust, Chamber of Commerce, or other organized groups. There is no limit to the number of times the Discount Vision Program can be used.

Customized Vision Benefit Programs
We create customized plans for groups with specific bid specifications, or benefit designs. Whatever your vision benefit program requirements are, we can provide you with the plan that suits your needs.
Vision Plan Materials
We provide employers with benefit summary brochures, participating provider directories, and claim forms for their employees who enroll in the vision plan. Participating provider listings and claim forms may also be obtained from this website.
Provider Selection
Patients may select an eye care specialist from the participating provider directory or from this website. Providers are arranged by state, county, city, and specialty.
Claim Form
To confirm enrollment, the patient completes Part 1 of the claim form and submits it to the provider at the time of the appointment. Claim forms may be downloaded from this website by patients and participating providers.
Eligibility Verification
Participating providers verify eligibility through this website or the MES toll-free number. They render the appropriate services, complete the examination and/or eyewear portions of the claim form, and submit directly to MES. The patient is not responsible for claim form submission when services are rendered by a participating provider.
Patient Responsibility
The patient is responsible for any deductible and/or co-payment, as well as any charges for services and materials not covered by the vision plan.
Non-Participating Provider
If services are rendered by a non-participating provider, the patient is responsible for submitting the claim form to MES, along with an itemized bill and a copy of the prescription. Payment will be made to the employee according to the schedule of allowances for non-participating provider services.
